Survey Instructions
The survey typically takes about 5 minutes to complete. There are 10 statements and 1 open-ended questions for a total of 11. Survey statements will be evaluated on a 1–4 scale:
- 1 means NO — you DISAGREE 100% with the statement
- 4 means YES — you AGREE 100% with the statement
- Anything in between means you don’t completely disagree or agree
Please read the statements carefully as some of them might be worded negatively, so a 4 score on one of these means you agree with the negative position that’s stated.
If you don’t have enough information to complete a statement just leave it blank and advance to the next one.
Use the arrow button on the bottom of the screen to navigate to the previous question or statement. Do not attempt to use the browser back button, as that may take you away from the survey.
At the end of the survey, you will also see an open-ended question in which you can include any additional information which you believe is relevant to the purpose of the survey.
